Willingness to work with individuals with physical and developmental disabilities mental illness. Must be capable of oral and written communication in English with individuals and fellow employees; along with the ability to read, implement, and document services and outcomes according to established guidelines.

Requires full range of body motion includinghandling and lifting individuals, manual and finger dexterity and hand-eye coordination. Requires standing and walking for frequent periods of time. Requires frequent bending, stooping, and stretching. Required to perform CPR and First Aid to American Red Cross standards during an emergency and during routine annual training and certification for CPR and First Aid. This may require staff to be on their hands and knees for an extended period of time to satisfactorily meet the standards of the techniques. Occasionally lifts and/or moves items weighing over 50 pounds. Requires corrected vision and hearing in normal range. Requires working under stressful conditions or working irregular hours.

Occupational Exposure Notification:

This position involves tasks which may reault in expsure to blood, body fluids, or tissues, or supplies or equipment contaminated with blood, body fluids or tissues.

The following list includes but is not limited to those tasks during which you are likely to come into contact with blood, body fluids, or tissues:

  1. Feeding tubes
  2. CPR
  3. Accidents/Injuries
  4. Personal Hygiene tasks
  5. Cleaning the whirlpool
  6. Housekeeping task
  7. Catheter care

Responsibilities: 

  • Reports all maintenance issues, accidents, and vehicle damange to Fleet Service Manager.
  • Completes all route, driving and vehicle documentation in writing and electronically required for billing and payroll reasons.  Ensures all billing documentation is complete and accurate. Must submit route sheets daily to department manager for audit.
  • Completes medical appointments / outings / behavioral interventions / special event transportation / personal care for individuals as assigned.
  • Completes activity coordination to meet person-centered plans, as assigned. 
  • Completes new driver training, Defensive driving course, comptency tests and continued education through the transportation department.
  • Assists with all direct care services to individuals including personalcare, toileting, meal preparation, feeding, transferring and all aspect of the IPP/ISP.
  • Maintains GPS and billing documentation for assigned routes
  • Utilized all routing, optimization software.  Maintains equipment in working order.
  • Accepts schedule and vehicle changes due to individual needs.
  • Responsible for submitting gas card receipts per use.
  • Responsible for the transportation of participants for all transportation contracts as assigned.
  • Accurately follows emergency procedures on the road to ensure safety of all individuals.

 

Requirements/Qualifications:

  • High school diploma or equivalency thereof
  • Ability to participate and pass the Medication Administration Course.
  • Valid driver's license with acceptable motor vehicle record.
  • Proof of personal auto insurance liability.
  • Access to personal vehicle (in good working condition per inspection) for transportation of individuals served.
  • Acceptable BCI and/or Criminal background check.
  • Willingness to work flexible hours, which includes additional hours and other days/shift hours if necessary.
  • Negative TB test (or chest x-ray)
